Create a folder for the hospital discharge papers, birth record, vaccination card and appointment notes. Ask the hospital what has been submitted for birth registration and which local registrar handles the birth.
- Keep the hospital’s registration reference or acknowledgement, if provided.
- Check spelling and birth details against the hospital record.
- Write down the next appointment and the contact details supplied by the care team.
First month: follow up on documents
Confirm the registration status through the responsible local authority. Ask for the document list and current process for your case before applying; requirements can differ for hospital births, home births and corrections.
- Keep relevant parent identity/address documents ready for the registrar’s requirements.
- Retain application acknowledgements and payment receipts.
- Review the issued certificate and ask the registrar about corrections if needed.
Through the first year: organise appointments
Keep the same immunisation card and bring it to each visit. Ask the care team to record the next appointment and explain what was given. If a visit was missed, ask the clinician about the next steps rather than changing the schedule yourself.

Start with clothes, nappies and cleaning supplies that fit your household’s chosen routine. Check what you already have and make space for laundry and changing. Keep a separate later list for larger sizes and optional purchases; feeding and care equipment choices should follow your care team’s advice.
